Referral Form For Professionals

If you are a professional and would like to refer a patient, client or service user to the NHS Talking Therapies service, please read the following information and use the link below as appropriate.

NHS Talking Therapies provides psychological support to people experiencing common mental health problems such as Anxiety and Depression, where this may include, but is not limited to, problems such as PTSD, OCD, Panic Disorder, Social Anxiety and Phobias.

Talking Therapies are generally not recommended at the time of an acute mental health crisis. If you have any concerns about the patients safety, please do not complete this referral. Instead, for Coventry and Warwickshire patients, please refer to the Mental Health Access Hub at 08081 966 798 or For Solihull patients, please refer to Single Point of Access at In the case of emergency, please contact 999.

Please note that Coventry, Warwickshire and Solihull NHS Talking Therapies is only able to accept referrals for patients with a registered GP in one of those specified areas. Patients with a GP outside of those areas should access their local NHS Talking Therapies service.

Please also note that NHS Talking Therapies can only accept referrals for patients aged 16 or over. Should you wish to refer a patient under the age of 16 for psychological support, please contact the Rise (Child and Adolescent Mental Health) via their website at or by calling 0300 200 2021. For Solihull patients, please contact Solar at or by calling 0121 301 2750.

Please ensure that consent has been obtained from the person before making this referral. If you have been unable to gain consent, either discuss with the patient before proceeding or feel free to pass on the self-referral information – patients can self-refer via this website or by calling on 0247 667 1090.
